Puppy Adoption Form
We require all potential new dog/puppy parents to fill out our dog/puppy adoption form.

This gives us a better idea of your lifestyle, family, living situation etc.


We take placing our dog/puppies VERY seriously. All puppies are placed in homes on a spay/neuter contract. A dog/puppy is a LIFETIME commitment.

Our Dog/Puppy Placement Requirements:

We have given all of our dogs/puppies a lot of love, proper care, attention, hugs and kisses!

So our major requirement is that you provide a safe, loving forever home and you will give our babies lots of love and attention!

Shih Tzu's are very social dogs and love lots of attention. This is one of the reasons they make such loving family pets. However it is not fair to them to be left alone all day and kept in a crate with no company. A way to solve this if you happen to work long hours is to get another dog or cat to keep them company.

But Shih Tzu do best when they have someone around most of the time to keep them company. So PLEASE think long and hard and do your research as a puppy is a LIFETIME commitment. You shouldn’t buy a puppy if you are not home enough to give them all the love and attention they deserve!

35. All of my dogs/puppies are placed in homes on a spay/neuter contract. One of the specifications in my contract is that if at any time during the pet’s life you are no longer able or willing to keep him/her then you are to contact me and the pet is to come back to me without a refund; THIS IS NOT NEGOTIABLE. This Shih Tzu is never to be given away, sold, placed in rescue, animal shelter or a pet store. This includes giving this Shih Tzu to a friend, family member, neighbor etc. Do you understand this?
